To register your child with Cheder Chabad, please complete the following three steps:

​

1. Fill out this form

   • You will need to provide a confirmation code.

   • To receive this code please fill out an application or call our office.​
      

2. Upload or email the following documents:

   • Copy of Birth Certificate​

   • Current Immunization records

   • Physical health form (Form DH-3040)
      

3. Pay the Registration fee ($300 until June 15, $500 After June 15)​
